ISUP–GUPS Joint Initiatives: Strengthening Collaboration Through Consensus and Shared Scholarship:

The collaboration between the International Society of Urological Pathology (ISUP) and the Genitourinary Pathology Society (GUPS) has gained exceptional momentum in 2025, marked by the publication of the first joint white paper and the successful completion of a large-scale consultation on intraductal carcinoma of the prostate (IDCP).

The joint ISUP–GUPS White Paper on Defining Indolent Prostate Cancer: Call for a Multidisciplinary Approach (Eur Urol 2025; 88: 8–10) represents a major step forward in addressing the pervasive problem of overdiagnosis in prostate cancer. Rather than focusing narrowly on the semantics of renaming ISUP Grade Group 1 disease, the paper proposes a pragmatic, evidence-based framework to distinguish indolent from potentially progressive cancers. It defines indolent prostate cancer as a low-volume, well-differentiated acinar neoplasm confined to the gland that would not shorten life expectancy if left untreated. The document stresses that its optimal recognition cannot rely on histology alone but requires coordinated input from radiology (particularly mpMRI), clinical parameters, and molecular profiling. By delineating the conceptual difference between clinically insignificant and indolent prostate cancer, it calls for a new, multidisciplinary model in which pathologists, radiologists, and urologists share diagnostic responsibility to minimize overtreatment while safeguarding oncologic safety.

In parallel, a large joint GUPS–ISUP expert consultation on intraductal carcinoma of the prostate (IDCP) reached completion this summer. Co-chaired by Rajal B. Shah and Glen Kristiansen, the project involved 32 expert panelists and more than 200 participants at the Boston consensus conference during USCAP 2025. Using a modified Delphi process, the group reconciled prior differences between GUPS and ISUP regarding IDCP terminology, grading, and management. Consensus was achieved on nearly all major issues: IDCP should be diagnosed according to the Guo and Epstein criteria (with basal cell IHC in difficult cases), the term “AIP, suspicious for IDCP” should replace ambiguous borderline diagnoses, and the IDCP component should be incorporated into Gleason grading whenever associated with invasive carcinoma, irrespective of Grade Group. Importantly, active surveillance is deemed inappropriate for cases of GG 1 carcinoma with IDCP, and such cases should be reviewed in a multidisciplinary setting that includes mpMRI assessment. The final manuscript—The GUPS–ISUP Joint Expert Consultation Recommendations on Intraductal Carcinoma of the Prostate—is now in submission and expected to appear later this year/beginning of 2026.

These two major undertakings exemplify the growing integration between ISUP and GUPS: evidence-based consensus building, open international participation, and an emphasis on bridging pathology with clinical management. A third collaborative initiative, a joint ISUP–GUPS project on testicular sex-cord stromal tumors, led by Andres Acosta and Dan Berney, has already been launched and follows the same structured methodology.

Together, these activities underline a shared commitment to harmonize diagnostic standards, reduce interpretive variability, and foster a global, multidisciplinary dialogue that benefits both pathology and patient care.”

Vienna ISUP Multidisciplinary Expert Consultation Consensus Conference:

On September 11th 2025, the ISUP held a Multidisciplinary Expert Consultation Conference on the Evaluation of Treatment Effects in Prostate and Bladder Cancer at the Josephinum venue in Vienna. The half day meeting was well attended with excellent presentations and informative discussion. The resulting manuscripts are anticipated mid-2026. We thank our sponsors (Zeta Corporation and Daiichi-Sankyo) and acknowledge the help of our colleagues at the Department of Urology and Comprehensive Cancer Center at the Medical University of Vienna for all their assistance in hosting this event.

Warm regards,

Liang Cheng, MD, MS
